The PDF patient consent form can be completed electronically. Parents and carers should open the document using Acrobat Reader software and then use the mouse to click on the fountain pen icon, which should appear at the top of the document, as part of the toolbar. This will allow the person completing the form to tick the boxes and sign their name at the bottom of the page.
